Care Plans + Guidance + Support

Care Plans Plus is a private care management company owned and operated by licensed and credentialed social workers skilled at conducting assessments, developing care plans, solving problems, monitoring wellness, providing education and advocacy, coordinating medical and psychological care, engaging in legacy work, and offering comfort and support for those dealing with life limiting illness and end-of-life.  As mental health counselors we are also trained in crisis intervention, mental health assessments, harm reduction, suicide risk reduction, and providing ongoing emotional support for elders, caregivers, and family members.  We care deeply about the wellness of our community's elders and we are committed to helping your aging loved one to live their best life possible.

Ethics

We follow the National Association of Social Workers Code of Ethics, which is available here. The Social Worker's Code of Ethics is rooted in the professions time-honored core values of service, social justice, dignity and worth of the person, importance of human relationships, integrity, and competence.

Standards of Practice

 

We adhere to the standards of practice for social work case managers as determined by the National Association of Social Workers which summarizes our work as follows:  

"The primary goal of case management is to optimize client functioning and well-being by providing and coordinating high-quality services, in the most effective and efficient manner possible, to individuals with multiple complex needs."
