Meet Our Founder

Hello! My name is Sue-Marie Casagrande.  I founded Care Plans Plus as a result of my desire to serve the seniors in my local community. I believe it takes a village to ensure our elders receive excellent care that is delivered with respect for the whole person, including their hopes and dreams for the future. I practice dignity conserving care that is person-centered, which means I listen closely to the needs and desires of your aging loved one while always putting safety first. I am committed to providing our seniors on the Olympic Peninsula with the very best options for aging well, which leads me to participate with other professionals that share my commitment to our area's aging population. I'm able to provide a local set of eyes and ears, combined with years of experience in assisting individuals in dealing with challenging circumstances due to medical and psychological issues and conditions.

I am qualified to conduct assessments and prepare care plans that measure independent functioning as determined by the Center for Medicare and Medicaid. Care plans can be required for estate planning purposes. They also help provide a guide from which family members and caregivers can work off of to create a comprehensive system of care for your aging loved one. Having a good plan and sustainable system of care in place can dramatically improve an senior's quality of life and ability to remain at home to aging in place.

I also help with referrals to local and statewide senior agencies, medical providers, and local home care companies. I can assist at hiring private caregivers for your elders. I can also help you and your elder cope with the stress of aging and illness. I look forward to meeting you and helping your aging loved one to live their best life possible!
